which code did you use ? ubuntus or debians
i *believe* i encountered similar odd failures which were caused by debconf priority.. eg a question wasn't shown which made my modifications fail

when do you run it ? which sequence number does yous udeb have in menu ? might be that the cd iwsnt mounted at that point in debian
in ubuntu its run dircetly after pkgsel and before the cd gets unmounted
cd is mounted, but it seems like lenny's d-i doesn't mount it in /target ... or if it does, it mounts it as /media/cdrom ...

btw, I did some debuging on italc and managed to get a backtrace for that "double free" crash some people are getting. It looks like a x11vnc bug and not an italc one.
I sent the trace to Tobias as he also knows x11vnc upstream, let's hope a fix will be found ...

the problem of iTalc is that it's using the devel release of x11vnc, so it's more likely to contains bug (but Tobias seems to need it for some features used by italc)

vagrantc, btw, what would you think about a bzr based lts.conf backend ... keeping the single hosts in separate files and merging the lts.conf from these files ... so you can roll back single changes to single hosts or the complete file
some script i.e. editltsconf would then run your favorite $EDITOR and do the bzr magic before and afterwards
